The form asks for your name, email address, optional telephone number, preferred reply method, enquiry category and an optional general message. It also records your acknowledgement of this information and your request for a reply.
How the form works
Submitted details are stored in a private enquiry inbox in this WordPress website. Access through the website is restricted to administrators. A notification email tells the configured recipient that an enquiry has arrived and links to that inbox; the notification does not include your submitted details.
The team uses your details to review and respond to your enquiry. Please do not send medical history, medication details, documents or information about another person through this form.
Security and retention
The form uses a security token and spam controls. Temporary hashed values derived from an email address and connection IP address are used to limit repeated submissions. These controls expire after a minute or an hour, respectively.
Enquiry records remain in the inbox until an administrator deletes them. Hosting logs, backups and email systems may have separate retention arrangements. Ask the team about its current retention policy if you need more detail.
Your privacy questions
You can ask to access or delete an enquiry, correct your contact details, or discuss how your information is used. Choose “General enquiry” on the contact form and include only what is needed to identify your request. The team may need to verify your identity.
